Investigations
Investigations that identify the root cause of the incident, injury or near miss provide the employer an opportunity to make systemic changes to the overall benefit of the OH&S Program and worker safety. REC understands that if you don’t know what your gaps are, you can’t grow, change and improve.
We want to work with your JHS Committee to delve deeper into your reports to determine if they were caused by:
Gaps in the overall system of Risk Assessments and Safe Work Procedures leaving room for error.
Gaps in the communication system between management and workers regarding safety expectations.
Behavioural trends leading to dangerous shortcuts and disregard of safety procedures.
Training shortfalls or Human Error.
Regardless of the cause, it’s important to address it to reduce the fear of reoccurrence and maintain a strong safety morale. There should not be fear in identifying weaknesses within your programs, as it provides an opportunity for growth. Your JHS Committee and Leadership Team require education and training that teaches:
The differences between a Preliminary and Full Investigation Report for submission to WCB, and the purpose of both.
An unbiased viewpoint that assumes a systems error rather than a worker error.
Types of questions to ask that dig into the root cause.
How to follow-up on evidence and follow through on additional questions.
How to develop the action plan to address deficiencies and ensure leadership implements.
